If you have a protected identity, you should download the form here and not use the online application.
You can apply for an evaluation there, whatever the level of your qualification. You can also apply for several evaluations in the same application.
If you don’t want to, or should not use, the online application (e.g. if you have a protected identity) the application form are available here:

Please note that if you use the application form you can only apply for one education per form.
